Awards will be based on the points accumulated from purchases from Pro Maintenance Supply, Inc. Rebate Forms must be received by fax or email. Please allow 5-7 business day to process. This form must be completed by an authorized company representative to be valid Only Purchases of Pro Maintenance Supply products qualify for the Awards Program Any misrepresentations or fraudulent information disqualifies the claim. Pro Maintenance Supply distributors, dealers, resellers and their employees are ineligible. Only purchases by customers from a Pro Maintenance Supply reseller in the 50 United States are valid. Offer void where prohibited, taxed, or restricted by Federal, State, Provincial or local law. Pro Maintenance Supply reserves the right to request additional information regarding this claim and the right to confirm identification. Pro Maintenance Supply understands companies have different policies regarding rebates and awards. It is the sole responsibility of the claimant to adhere to his or her own company policy. Any submission with an invalid or undeliverable address will be rejected. Pro Maintenance Supply is not responsible for claims lost stolen damaged, illegible, misdirected or delayed in the mail. Please keep copies of all materials submitted. No claims against lost materials will be evaluated unless accompanied by proof of receipt of the original claim by Pro Maintenance Supply. The claimant waives any and all claims against Pro Maintenance Supply relating to this offer. Any tax liability or tax reporting or compliance obligations associated with the accrual, use or redemption of any Pro Maintenance Supply, Inc. points is the sole responsibility of the redeeming company, its employees or agents. Points may accumulate in a Pro Maintenance Supply, Inc. account for up to 1 year. However, the points may not be redeemed for cash. Pro Maintenance Supply reserves the right to cancel or modify this program at any time.
